REGISTRATION & TUITION INFORMATION
SCHOOL YEAR 2008-2009
ENTRANCE REQUIREMENTS
Saint Juliana Catholic School has an open
admission policy. Enrollment does not discriminate on the basis of race,
color, religion or national origin. Saint Juliana Catholic School entrance
requirements for Pre-Kindergarten through eighth grade are as follows:
Pre-Kindergarten "3" Program
We are now accepting
applications for the new Pre-Kindergarten 3 program for the 2008-2009 school
year. If you have a child who will be three years old by September 1, 2008
and would like more information about the program, please contact Mrs. Diana
Fernandez at 655-1922. We are offering a morning program and a full-day
program with the flexibility to meet your child's needs.
Pre-Kindergarten
– 4 year old program: Child must be four (4) years old by September 1, 2008
and be completely toilet trained. School will notify parents on or before
April 1, 2008 in order to arrange parent interview and student screening.
Kindergarten:
Child must be five (5) years old by September 1, 2008. School will notify
parents on or before April 1, 2008 in order to arrange parent interview and
student screening.
SCREENING RESULTS WILL BE REVIEWED BY THE
PRINCIPAL AND NOTIFICATION OF ACCEPTANCE WILL BE MAILED.
First Grade through Eighth Grade:
An interview with the parents and enrolling student may be required by the
principal to review student academic progress and classroom behavior. In
order to complete your child’s application, we will need the following
information from your child's previous school:

Most recent academic report card

Most recent standardized test
results
-
Letter of recommendation from the
previous school's administrator
-
Letter of recommendation from the
previous school's
teacher
We also require the
following documents to be included with the child's application

Official birth certificate
-
Baptismal Certificate and First
Communion date
-
Copy of social security card
The completed application with the above
mentioned paperwork must be submitted for review before applicant can be
considered for admission.
REGISTRATION INFORMATION
I.
Registration Fees:
A registration fee of $175.00 per child is
due at the time of registration.
THIS FEE IS NON-REFUNDABLE.
II.
Currently Enrolled
Student Registration Information:
Current family registration will be held
from January 7, 2008 through
February
8, 2008.
The
following forms must be completed in their entirety and
returned
at the time of registration. (for currently enrolled students)

Tuition Payment Preference Form (yellow)
Only those families whose tuition
payments are current through January
will have their registrations processed.
Currently enrolled families who
are not re-registered by February 8, 2008
will lose their priority status.
New family applications will be accepted
beginning Monday, February
11, 2008.
III.
New Student Registration
Information:
New family registration will begin on
February 11, 2008.
The following documents are required at the
time of registration:
1. Family Information
Registration Form (green)
2. Tuition Payment Preference Form
(yellow)
3.
FACTS Tuition Agreement with a void check
4.
Family Acknowledgement Form (blue)
5. Florida Certificate of Immunization,
Form DH 680 (blue form)
This is the only document that schools are
permitted to accept
as proof of immunization.
6. Medical examination by a Florida physician on form DH 3040
(yellow form). Exam must be taken after May
31, 2008
7, Legal papers, i.e. custody
arrangements, if applicable.
8, I-20 Immigration papers completed, if
applicable.
TUITION FEES AND POLICIES FOR 2008-2009 SCHOOL YEAR
1.
Tuition Fees:
For Saint Juliana Parish families,
supporting status requires weekly attendance at Sunday mass and financial
contributions to the parish. For all other Catholic families, a written
statement showing financial support will be required from the Pastor of your
home parish. Supporting status in these parishes will be determined by your
home parish Pastor. The total of your annual support through the use of
offertory envelope contributions and volunteerism are the major determining
factors.
All non-supporting families, and those
Catholic families not meeting the requirements for support, will be placed
in the non-supporting category and charged tuition accordingly.
In order to assist your Pastor in
determining your support status, we encourage the use of checks or the
offertory envelopes. Each parish has recorded all contributions from checks
and/or envelopes for several years. The Internal Revenue Service requires
your parish to be able to provide you with written documentation of your
annual contributions when they exceed $250.00. For cash contributions, the
use of the offertory envelope is required to allow them to fulfill this
requirement.
A limited amount of financial aid is
available for qualifying families. Contact the office for information.
II.
Tuition Policies:
All tuitions listed in the
tuition fee table are based on a twelve (12) month
period.
Tuition payments begin in June 2008.
Attached you will find
the Tuition Payment Preference
Form which must be returned with the
registration paperwork.
III.
Other Fees:
Student Insurance (NOT OPTIONAL) -
$20.00 per student. This fee will appear on the book rental bill.
Home and School Association Family Dues –
Family dues are payable by separate check to Home and School Association (H.S.A.)
and due in August 2008. This will appear on the book rental bill.
ADDITIONAL REGISTRATION INFORMATION
I. Uniforms:
All students are
required to wear the appropriate uniform as described in the
Saint Juliana
School Handbook for Parents and Students.
Uniforms must be
purchased directly from the uniform company. Uniforms
are not
available for purchase from the
school.

FAMILY VOLUNTEER PROGRAM
Saint Juliana Catholic School requires all
families to donate fifteen (15) hours of volunteer time to assist in
fundraising events and other school personnel needs. If a family is unable
to complete hours of volunteer time or neglects to log their volunteer hours
during the school year, the family will be charged $10.00 for each hour not
completed. If a family is unable to donate any time during the school year,
a payment of $150.00 will be required.
Each family is also responsible for either
purchasing or selling an amount equal to $100.00 for the annual auction
raffle held the night of the event.
EXTENDED SCHOOL DAY PROGRAM
Saint Juliana Catholic School is pleased to
offer an Extended School Day Program for our parents. This program is only
available for Saint Juliana School students.
The Extended School Day Program is held in
the Saint Juliana School cafeteria from 3:00 p.m. to 6:00 p.m. and offers
supervised homework time, outside play, games and activities. There are no
additional charges on half days of school for students who attend the
program on a monthly basis.
An application fee is due at the time of
registration in the Extended School Day Program. Payments are due on the 1st
of each month.

For St. Juliana Parish, the main elements
for being considered a 'registered, active, and supporting Catholic' family
have been regular (every Sunday and Holy day) attendance at Mass and a total
of at least $650.00 in offertory donations during the calendar year of
2008-2009.
Each parish has its own policy regarding the
Catholic Tuition Rate (so members of other parishes need to check with their
parish office).Written verification by the pastor of the Church at which the
family is registered must be received at the time of registration for
supporting rate to apply.

Volunteer Hours:
15 hours per family (of which 4 hours have to be served at the lunch
program, 2 hours at Shamrock, Shindig, 3 at the Auction) or $150.
